Testosterone is a controlled substance regulated by the FDA and requires prescribing by a licensed healthcare provider with an active DEA registration—which I hold as a Nurse Practitioner. As such, ongoing clinical oversight and follow-up care are not optional—they are legally and medically required.

Testosterone Therapy Requirements

To ensure your safety and achieve the best possible results, the following protocols must be followed:

Follow-up Appointments:
You are required to meet with your provider at least every 2 months for follow-up visits. These check-ins are essential to assess symptoms, make dosage adjustments, and ensure overall well-being.

Lab Monitoring:
Comprehensive lab work is required every 2 months until your hormone levels are stable. This helps us make informed decisions about your care and monitor for any potential side effects.

Pharmacy Coordination:
We will send prescriptions to the pharmacy of your choice. Note: All testosterone prescriptions are from licensed, commercial or compound pharmacies only—no research-grade or unregulated sources.

Why Don’t You Offer Hormone Pellets?

At NP Empowered Wellness, we prioritize safe, flexible, FDA Approved and evidence-based hormone therapy. While pellets are popular in some practices, we do not offer them due to:

  • Lack of dosage flexibility (once inserted, they can’t be adjusted)

  • Higher risk of side effects from hormone spikes

  • Limited ability to personalize treatment over time

We believe in meeting regularly with our patients, monitoring labs, and adjusting treatment as needed to ensure optimal results.

Micronized Progesterone

  • Progesterone is a key hormone produced by the ovaries. It plays a vital role in regulating the menstrual cycle and supporting early pregnancy. As women approach perimenopause and menopause, progesterone levels decline, which can contribute to a variety of symptoms.

  • Micronized progesterone is a bioidentical form of progesterone—meaning it’s structurally identical to what your body naturally produces. It’s derived from plant sources and processed into tiny particles (micronized) to improve absorption when taken orally.

    Benefits of Micronized Progesterone:

    • Better tolerated than synthetic progestins

    • Supports sleep and mood

    • Protects the uterine lining when using estrogen

    Compared to synthetic versions, micronized progesterone has a more favorable effect on cholesterol, breast tissue, and mental health.

  • Typically prescribed in capsule form, micronized progesterone is taken orally at bedtime—often ranging from 100–200mg, depending on your needs.

    Instructions:

    • Take at night — it can have a calming, sedative effect

    • With or without food — but taking it with food may enhance absorption

    • Consistent timing is key for hormonal balance

    Your exact dose and schedule will be tailored based on your symptoms, labs, and whether you're also using estrogen.

  • Yes—commercially manufactured micronized progesterone (such as Prometrium) is typically formulated in a base of peanut oil to help stabilize the hormone.

    If you have a peanut allergy or sensitivity to fillers or dyes, don’t worry—we offer compounded progesterone made in specialty pharmacies without peanut oil, dyes, or unnecessary additives. These custom formulations are tailored to your individual needs and sensitivities.

    👉 Always let your provider know about any allergies so we can ensure your hormone therapy is both safe and effective.

Why Don’t You Recommend Progesterone Cream?

While progesterone creams are widely available, they are not recommended at NP Empowered Wellness for several important reasons:

❌ Inconsistent Absorption

While progesterone is often marketed in over-the-counter creams, and made by compound pharmacies, it's important to know that the progesterone molecule is too large to be effectively absorbed through the skin in therapeutic amounts. This means that most topical formulations—which have not been proven effective-may not provide consistent or reliable hormone levels.

❌ Does Not Protect the Uterus

When using estrogen therapy, adequate progesterone is essential to protect the uterine lining. Creams do not provide protection against estrogen-induced endometrial thickening or cancer.

    • Liver disease

    • History of stroke or heart attack

    Even if you have a medical history that raises concern, we take a personalized, risk-based approach. In many cases, non-estrogen alternatives or local (vaginal) estrogen may still be safe and effective options.

  • Vaginal estradiol is a low-dose form of estrogen therapy designed to treat genitourinary symptoms of menopause (GSM), such as vaginal dryness, irritation, burning, painful sex, or urinary frequency. It’s applied locally to the vaginal tissues.

  • Genitourinary syndrome of menopause (GSM) includes symptoms like vaginal dryness, pain during sex, and urinary issues due to low estrogen levels in menopause. About 50% of postmenopausal women experience these changes, leading to discomfort and affecting their quality of life.

  • Vaginal estradiol:

    • Restores vaginal tissue thickness, moisture, and elasticity

    • Improves comfort during intimacy

    • Reduces vaginal itching, burning, and dryness

    • Supports urinary tract health, helping reduce frequency and urgency

    These symptoms are common during and after menopause due to estrogen decline—and vaginal estradiol is one of the most effective ways to treat them.

Directions
Before mixing the large and small pack in a small bowl, massage the small packet to soften it. After mixing thoroughly with mixing stick for about 1 minute, push the gel together in the bowl. Use applicator to pull up the gel. Insert into vagina and  on vulva. Apply 2/3 of the gel into the vagina. The rest apply to vulva. Leave on for 45 minutes minimum. You can leave on overnight. Rinse out of vagina by filling applicator with water. Do this about 3 times. Wash off the vulva. Pat dry.

Do the first 5 to 10 treatments within the first month. Then you can maintain with monthly treatments.

  • Before combining the contents of the larger and smaller packets in a small bowl, gently massage the smaller packet to ensure its smoothness. Once thoroughly mixed with a stirring stick for approximately 1 minute, consolidate the gel in the bowl. Use the applicator to collect the gel and administer it both inside the vagina and on the vulva. Apply around 2/3 of the gel inside the vagina, using the remainder on the vulva. Allow it to remain for a minimum of 45 minutes; overnight application is also an option. To rinse the vagina, use the applicator filled with water, repeating this process about 3 times. Cleanse the vulva area thoroughly and pat dry. Initially, aim to complete the first 5 to 10 treatments within the initial month. Subsequently, maintenance can be achieved through monthly treatments. For optimal results, consider performing a treatment either daily or every other day.
